Form 5500 / Summary Annual Report (SAR) Preparation

All welfare benefit plans are required to file a Form 5500 each plan year as long as there are 100 or more participants as of the beginning of the plan year. All forms and schedules must be filed by the last day of the 7th calendar month after the end of the plan year.

OneSource prepares your Form 5500. We will request all Schedule A data from the carriers and forward 2 copies of the completed Form 5500 and Schedules to you for your review, signature and filing.  

Two copies of the Summary Annual Report (SAR) will be included in your Form 5500 package.  The SAR should be made available and/or displayed on your intranet.  If you receive a written request for copies of the SAR or Form 5500, you must respond within 30 days of receipt otherwise penalties apply.
