Medicare Part D Disclosure Notice

Any employer that provides prescription drug benefits through a group health plan must provide a Medicare Part D Disclosure Notice annually to each covered person who is, or who is about to become eligible for Medicare. 

Baystate Onesource will notify and remind you mid-October every year to distribute the Part D Disclosure Notice.  A sample cover letter and Disclosure Notice will be included so you can distribute to all medical plan participants.
